Every care relationship begins with understanding the person. We learn about the client\u2019s needs, routines, preferences, goals, home environment, and the support already in place. From there, we build a personalized care plan, thoughtfully match the caregiver, and stay involved after care begins.<\/p>\n<\/div>\n

Care Needs Assessment<\/h3>\n

\n The assessment gathers information about the client’s health history, cognitive and physical status, assistive devices, activities of daily living, household routines, safety concerns, preferences, schedule, and support system.
