Every care relationship begins with understanding the person. We learn about the client’s needs, routines, preferences, goals, home environment, and the support already in place. From there, we build a personalized care plan, thoughtfully match the caregiver, and stay involved after care begins.

We begin by learning about the client’s daily routines, abilities, preferences, mobility and safety concerns, assistive devices, ADL/IADL support needs, and other information relevant to planning non-medical home care.

Best Care Conversation

We talk with the client and, when appropriate, their family or representative about what excellent care means to them, what is most important, and how they want support provided.

Personalized Care Plan & Pricing

We recommend a care plan based on the information gathered, including the services, schedule, level of assistance, and applicable pricing. The plan is reviewed with the appropriate parties before care begins.

Caregiver-Client Matching

We consider more than availability. Care needs, personality, communication style, routines, preferences, and other compatibility factors are considered when selecting a caregiver.

Once the care plan and caregiver assignment are in place, services begin according to the agreed schedule and care plan.

We stay engaged after care begins through communication, feedback, check-ins, and adjustments when needs, preferences, schedules, or circumstances change.

What happens during the initial home care assessment?

The assessment gathers information about the client’s health history, cognitive and physical status, assistive devices, activities of daily living, household routines, safety concerns, preferences, schedule, and support system. The purpose is to understand what the person needs help with and what they want to continue doing independently.
What is the “Best Care Interview”?

Care By An Angel uses this conversation to ask the client and/or authorized representative what they personally consider excellent care. Instead of assuming that every family wants the same thing, the agency identifies preferences, expectations, communication style, routines, and priorities that should shape the care plan.
When will we know the price of care?

Pricing is presented with the recommended care plan after the agency understands the client’s needs, schedule, location, and service requirements. The client or responsible party should have an opportunity to review the proposed services, schedule, pricing, and agreement before care begins.
How is the caregiver chosen?

Care By An Angel considers the caregiver’s qualifications together with the client’s needs, personality, preferences, communication style, schedule, and expectations. The agency believes rapport and chemistry are important to a successful ongoing care relationship.
What if the caregiver is qualified but my parent does not connect with them?

Tell Care By An Angel. A technically qualified caregiver may still not be the right personal match. The agency welcomes feedback and can review the concern, coach where appropriate, or adjust the caregiver assignment to better fit the client’s needs and preferences.
How often is the care plan reviewed?

Care needs can change at any time, so families should not wait for a crisis to report new concerns. Care By An Angel uses ongoing communication and check-ins to evaluate satisfaction, caregiver fit, and changing needs. Formal review requirements may also vary by service type, payer, and state.
Can care hours be increased or reduced later?

Yes. If the client’s needs or family support changes, the schedule can be reviewed. Changes should be communicated to Care By An Angel so the service plan and agreement can be updated appropriately and caregiver scheduling can be coordinated.
